Chimney Trail Health Mobile Terms of Service

001 PROGRAM DESCRIPTION

Chimney Trail Health sends two separate categories of recurring text messages related to your participation in a Chimney Trail program. Neither category is marketing or promotional.

Operational messages. Messages may include Waypoint Kit notifications and reminders or notices related to program surveys. These messages are intended to support program participation, logistics, and customer service.

Waypoint check-in messages. Recurring, non-marketing encouragement texts that support your participation in your current Waypoint Kit program: reminders to use your kit materials, suggestions for setting practice goals, and prompts to revisit program activities. Check-in messages are sent from a separate phone number and are covered by a consent that is separate from your consent to operational messages.

002 MESSAGE FREQUENCY
Message frequency varies. Operational message frequency varies based on Waypoint Kit status and your specific program survey schedule. Waypoint check-in messages are sent approximately every two weeks for the duration of your program.
 
003 OPT-IN
You opt in to each category separately, by checking a dedicated consent box during Waypoint Kit program registration.
 
Providing your mobile number and checking the operational consent box means you agree to receive recurring operational text messages from Chimney Trail Health. Checking the separate Waypoint check-in consent box means you agree to receive recurring Waypoint check-in messages, which are sent from a different phone number.

Consent to operational messages is not consent to Waypoint check-in messages, and consent to either category is not a condition of purchase or of participation in a Chimney Trail program.

004 OPT-OUT
You can stop either category of message at any time by texting STOP in response to any message you receive from us, in a single step, with no further action required.
Because the two categories are sent from two different phone numbers, replying STOP stops the category sent from the number you reply to:
 
  • Reply STOP to a Waypoint check-in text and you will stop receiving check-in texts from that number. You will still receive your operational messages — kit notifications and program survey notices — and your program emails.
     
  • Reply STOP to an operational text and you will stop receiving operational texts from that number. If you have separately opted in to Waypoint check-in texts, those continue unless you also stop them.

You will receive a confirmation that you have been unsubscribed, and messages in that category will stop.
You can also ask us to stop sending you either or both categories of text messages at any time by contacting us at info@chimneytrail.com or via the 'Contact Us' form — tell us which texts you want stopped, or that you want all texts stopped, and we will honor your request.

If you want to join again, you can reply START to that same number, or opt in again through the original enrollment method or by contacting us via the 'Contact Us' form available on the website. Stopping Waypoint check-in texts never affects your ability to participate in the program.

005 CUSTOMER CARE
If you are experiencing issues with the messaging program, reply HELP to any message for more assistance, or contact us directly at info@chimneytrail.com. Replying HELP to a Waypoint check-in text returns information about the check-in program.

Text messages to and from Chimney Trail Health are not monitored continuously, and the messaging program is not an emergency or crisis service. If you are experiencing a mental health emergency or crisis, call or text 988 (Suicide & Crisis Lifeline) or call 911.

006 MESSAGE AND DATA RATES
Message and data rates may apply for any messages sent to you from us and to us from you. If you have questions about your text plan or data plan, contact your wireless provider.

007 CARRIER DISCLAIMER
Carriers are not liable for delayed or undelivered messages.

008 PRIVACY

009 CHANGES TO THESE TERMS
We reserve the right to change, modify, or otherwise update these Mobile Terms of Service at any time. Unless otherwise noted, any changes will be effective immediately upon posting, and we will update the effective date at the top of this page. If any changes are material, we will provide you with notice of such changes by posting notice on the website or by other means (such as email or text message).
